Direct debit request form
This form enables you to apply for Direct Debit on any ActewAGL accounts via bank account or credit card. As we do require your signature on this form please print and once completed return the signed form to ActewAGL.
To complete this form you will need:
- your account name
- the premise address
- a contact number
- your ActewAGL account numbers
- your financial institution information
- the Direct Debit type you wish to select
- the date for the first payment (not required for Direct Debit Full Pay).

Change of financial institution information form
This form allows you to change the bank account or credit card you are using for your Direct Debit. As we do require your signature on this form please print and once completed return the signed form to ActewAGL.
To complete this form you will need:
- the account name
- premise address
- a contact number
- your ActewAGL account numbers
- your financial institution information.

Change or cancellation form
This form allows you to cancel or change your Direct Debit application details. Note: to make changes to your financial institution information please use the Change of financial institution information form. This form can be submitted online.
To complete this form you will need:
- the premise address
- your email address or contact number
- all ActewAGL/TransACT account numbers which require the Direct Debit to be changed or cancelled
- the date for Direct Debit to be cancelled (not required when simply changing details).
